Wednesday, March 16th at 1:30pm. Join us for this free information event introducing the statewide Gatekeeper Program, presented by St. Luke’s Community Services. This program’s goal is to help clients remain independent and successful in their communities.
The Gatekeeper Program is a referral-based program that offers free short-term case management for adults (60+) who are experiencing mental health and behavioral challenges: which includes psychosocial assessments, information and referral for relevant programs and services, benefits screens, benefits application assistance, etc. The Gatekeeper Program also educates community members (especially those who regularly are in contact with older adults) to notice and identify seniors who are potentially at risk or in need of assistance due to cognitive or behavioral health challenges.
The event is free but space is limited – advance sign-up is required.
